It may be due to many reasons and it can be found by doctor if the reports are given regularly. There may be miscarriage due to hormones imbalance, or any accidental loss etc.

some more miscarriage reasons in detail.
Abnormalities In The Chromosomes
Miscarriages that occur because of problems in the chromosomes of the embryo are most common. In such cases, the miscarriage occurs within the first 13 weeks of pregnancy, sometimes much before the woman realizes that she is pregnant.
Hormonal Imbalance
If the body of the pregnant woman does not produce enough progesterone, a hormone that prepares the inner wall of the uterus to nourish a fertilized egg, the result is a miscarriage. In such cases, progesterone supplements should be given to the pregnant woman to avoid the occurrence of a miscarriage. An imbalance in hormone is also caused by diabetes mellitus or thyroid issues.
Ectopic Pregnancy
A faulty implantation of the fertilized egg is known as ectopic pregnancy. The commonest site for ectopic pregnancy is the fallopian tube although the embryo may start to grow in the ovary as well. Severe cramps and stomach pains are noticed during such pregnancies and the fallopian tube may be damaged beyond repair during the process of miscarriage as well.
Infections or Food Poisoning
Some types of viral and bacterial infections can lead to miscarriage or stillbirth. Infections generally pose a higher risk of pregnancy loss in the second trimester and later, but a few can cause problems in the first trimester also.Chitra Kumar

here are some more valuable points regarding miscarriages,
Miscarriages are also known as spontaneous abortions, as they can occur spontaneously without warning. In most cases, miscarriages occur before the end of first trimester, means before the 12th week of pregnancy. These miscarriages are known as early miscarriages. In some less common cases, miscarriages occur during the second trimester and are known as late miscarriages. A possibility of miscarriage in the third trimester is very rare.
Chromosomal abnormality is one of the most common reasons of miscarriages. About 70% of first trimester miscarriages result from chromosomal defects. Hormonal imbalance is also responsible for miscarriages in the first trimester. Hormonal imbalance is due to a deficiency of progesterone, which makes the uterine environment less receptive.
Late miscarriages in the second trimester of pregnancy are due to low-lying placenta causing bleeding, incompetent os (weakness of cervical muscles), hydramnios (excessive water) and lifting heavy weights.

yeah most of the miscarriages occurs only in 1st trimester so the pregnant women should avoid travel in the 1st trimester.
